Treatment Approach for T2b

Neoadjuvant gemcitabine-cisplatin (preferred) or MVAC followed by radical cystectomy. Trimodal therapy if unfit for surgery. 20-35% achieve pathological complete response (pCR) with chemotherapy; pCR predictive of improved survival.
